I can only really comment for Southwestern Ohio Departments (Cincinnati and Dayton areas)... but generally most departments run both Fire and EMS. There are several departments that run Fire and BLS with ALS done by a third party. The third party either runs directly on the FD's rig or responds to the scene, again, depending on the department. The vast majority of the FDs are ALS, but many will hire basics.
All that being said... There are equal oppurtunity for both Vollie and Career around the state, and many may allow you to just do EMS. I believe there is an all vollie EMS group near Dayton, but their name escapes me now.
On that note if you are just an EMT-B, and you want to transfer to Ohio... Ohio Basics can intubate (pulseless apneic), but that is the only skill above and beyond National Registry.

To the best of my knowledge, you'll be hard-pressed to find something similar to that around Michigan. About the closest would be the Genesee County Sheriff's Department, since they run Echo (ALS Non-transport) units, but you have to be a sheriff's deputy as well as a medic. I believe that every large city in lower MI (Detroit, Flint, Lansing, Saginaw) have FD run EMS, although Flint has cut back severely due to funding issues. They now run MFR engine companies and a single Echo unit. They used to have the echo, 2 or 3 ALS, and 2 BLS trucks.
There aren't enough high population density areas around here that support volunteer EMS. There are a LOT of services in low-density areas (like mine) but you can't call 150-200 calls PER YEAR exciting. Those services are usually run through the FD, but many do NOT require cross-training.

I'm not sure if you would be interested in the western part of the Lower Penisula but my brother-in-law works for private company called Pro-Med out of Muskegon. He is full-time and seems to enjoy it. I don't know all the particulars but go to www.promed.org and that might be able to help you out for some more information.

Up here in the THUMB I would say it's about 50/50,down by Detroit :shock: better have a bullet proof vest,although there is good parts and bad parts depends on what kind of action you are looking :roll: for.

In the Cleveland area, all ems is run by the fire depts. The City of Cleveland has a separate ems dept. responding to 911 dispatches. No transport from one facility to another or patient discharges. There are a lot of tough neighborhoods and the call volume is high. Plenty of fast paced excitement! They only hire full time and you get good city benefits and pension that are comparable to Fire and Police.
Most of the Dept in my area NW OH are volunteer with many, including ours using part- time crews for EMS. We run two people,usually at leastone is a medic, M-F from 8:00 am to 5:00 pm. We respond to all calls including fires, during the part time hours in our mini-pumper which is fully ALS equipped. All of our part timers are FF and EMT's and this unit also has a limited firefighting role. After 5:00 pm and on the week ends, one of our medics takes home our ALS Echo unit (fly car), and responds to all calls with the volunteers for backup and transport.
The City of Toledo, which we border is full-time and houses and staffs the County Ran ALS Life Squads in several of their stations. TFD responds an engine or rescue crew to EMS calls. Most of these are ALS also. There are three private ambulance companies in Toledo that do most of the BLS transports for the City, as well as hospital and nursing home transports.
I don't know too much about Michigan, but I do know that AMR operates in Monroe County.
